This application allows a business to request a license to practice in New York State as an:
Elevator Contractor – responsible for the design, construction, maintenance and repairs on elevators and conveyances, or
Elevator Inspection Contractor – responsible for the inspection and testing of elevators and conveyances
If you would like to apply for both an Elevator Contractor and an Elevator Inspection Contractor license, you must submit a separate application for each license.

Prerequisites
- Must provide proof that the legal name of the business is registered.
Requirements
For Elevator Contractor:- Must have an employee that holds a current or pending individual Elevator Mechanic license.
- Must have proof of appropriate insurances (Workers’ Compensation, Disability and Paid Family Leave Benefits, and General Liability with One Million Dollar minimum occurrence coverage).
For Elevator Inspection Contractor:
- Must have an employee that holds a current or pending individual Elevator Inspector license.
- Must have proof of appropriate insurances (Workers’ Compensation, Disability and Paid Family Leave Benefits, and General Liability with One Million Dollar minimum occurrence coverage).
